Document requirements

Identification:
The animal must be identified by a microchip that complies with the ISO 11784/11785 standard. A tattoo may also be accepted if it was made before 3 July 2011 and is clearly legible.

Vaccination:
The animal must be vaccinated against rabies. The vaccine must be administered no earlier than 12 weeks after birth and no later than 3 weeks before entering the Czech Republic.

Veterinary certificate:
It is necessary to obtain an international veterinary certificate that confirms the health of the animal and the fact that the necessary vaccinations have been carried out. It is issued by the state veterinary service no later than 5 days before the trip.
To import a pet, you must enter through one of the animal entry points. For example, these are Prague airport (Václav Havel Airport Prague) and Brno (Brno-Tuřany Airport).

If you have come to the Czech Republic for a long time

All dogs must be registered with the local government and pay a tax (1,500 CZK for one dog and 2,250 CZK for each additional dog)

Document requirements

Health certificate:
Not required. 24 hours before departure, you must get a stamp of examination by a veterinarian, if necessary, you can undergo treatment for parasites.

Certificate for departure:
Obecné veterinární osvědčení. Obtain at the municipal veterinary station SVS. You must appear in person (possibly without the animal), with the animal’s passport (it must contain a chip, vaccinations), with titers (if necessary) and with the passport with which you will travel. The cost of the certificate is 50 CZK.

The certificate is valid for 10 days for departure outside the country, then the certificate is valid until the end of the route, unless otherwise stated. That is, you need to receive the certificate 10 days or less before the start of the trip.

Rabies titers:
Needed for travel to a rabies-red country and return back to the EU, as well as to Israel, China, Turkey without return.
